Stage 6: Sustain improvement and spread

Once your improvement project has achieved its aim, consider how to sustain the improvement, and how you can spread the learning.

Sustainability excel tool

To review your project's sustainability and any actions required, complete the sustainability excel tool. This will highlight to you any areas where further actions are required to support ongoing sustainability of the improvement.

Spreading CQI

Use the Spreading CQI template to plan how your learning and improvements can be spread across the organisation.

CQI project poster

Use the CQI project poster to summarise your work for shared learning and celebration. Your QI advisor will meet with you to discuss the completion of your project and project poster completion. Please contact your QI advisor directly or the QI team via QI@wsh.nhs.uk.
