Pulmonary rehabilitation - information for health professionals

Referral information

Patients can be referred to our pulmonary rehabilitation courses by their consultant, GP, COPD nurse, respiratory nurse specialist, or physiotherapist.

Once a referral is received, the patient will be triaged and placed on a waiting list.

Please ensure

• Referral forms are completed with as much detail as possible, especially regarding medical history.
• The patient understands and agrees to attend pulmonary rehabilitation, committing to two sessions per week for six weeks.
• Patients meet the inclusion criteria before referring them.
• Incomplete referral forms may be returned, which could delay the patient’s place on a course.

• PR referral form via Evolve or DXS referral system
• Post to: East Suffolk and North East Essex NHS Foundation Trust, Care Co-ordination Centre, 5 Constantine Road, Ipswich, IP1 2DH

Alternative support if a patient is not suitable for a course

1. 1:1 assessment with an outpatient respiratory physiotherapist
2. Referral to community support services such as Feel Good Suffolk, Breathe Easy Support Groups, or Asthma + Lung UK
3. Referral to Wellbeing Suffolk
4. Referral to an active living class through Abbeycroft Leisure Centre
