Continence products referral

Any prescribed products are a contribution towards meeting the continence need of the child, parent's or carer's may need to buy additional products.
We do not provide pull up style products.
The child must be registered with a GP surgery within the Suffolk boundaries (Waveney is excluded as it is served by NHS Norfolk And Waveney Integrated Care Board).
All referrals for products in the area covered by East Coast Community Healthcare continence service need to be made to the service via the registered GP surgery.

It would be helpful if you could complete the continence promotion assessment
If a continence promotion assessment has been completed please have it ready in an electronic document as you will have an opportunity to upload it when completing the referral.
