It would be helpful if you could complete the continence promotion assessment (DO YOU HAVE A COPY OF THE MOST UP TO DATE VERSION WHICH WE COULD PUT THE TRUST LOGO ONTO?)
If a continence promotion assessment has been completed please have it ready in an electronic document as you will have an opportunity to upload it when completing the referral.
