Early intervention team

The Early Intervention Team (EIT) support and provide medical care to patients across west Suffolk. The team work within patient’s homes (who have a West Suffolk GP), as well as our Emergency Department and Acute Assessment Unit to prevent unnecessary admissions to hospital. 

The team are integrated across health and social care boundaries and utilise the voluntary sector.  This cross-disciplinary approach helps ensure our patients have the support they need it.

 EIT consists of:

  • community nurses
  • social workers
  • occupational therapists
  • physiotherapists
  • specialist paramedics
  • assistant practitioners
  • reablement support workers (carers)
  • British Red Cross support workers
  • Suffolk Family Carers link worker. 
  • The team also work closely with the Dementia Intensive Support Team and St Nicholas Hospice. 

Together, EIT provide a “one stop shop” to help individuals retain their independence and avoid hospital admission when it is unnecessary. 

 

Service provided

  • Urgent (same day) therapy assessment of mobility and function.
  • Urgent provision of loan equipment to help with everyday living.
  • Assessment and organising emergency re-ablement package of care and onward Social Services referrals.
  • Admission prevention and out of hours nursing intervention.
  • Emergency end of life interventions to enable someone to remain at home (equipment, care, nursing, therapy)
  • Rapid Intervention Vehicle – assessment and management from specialist paramedic and therapist.
  • Signpost & refer to onward community services

Location:

The EIT office is at West Suffolk Hospital, but we also work in the community.

 

Opening hours:

Therapists (Mon-Fri): 8.30am – 9.00pm (Weekends and Bank Holidays): 09:00 – 18:00

Nurses: 24 hours.
